Waiver Statement
By submitting this form online, I am representing and promising the following:
1. I am the parent or legal guardian of the student identified above (Student).
2. I acknowledge the following:
A. Engaging in Student activities at The Light KC, whether on site, traveling to other sites, or at other sites (Activities) includes personal risks of injury, property damage, and exposure to or contracting illness.
B. While some adult supervision will occur, that supervision will not be continuous as those supervising will also be attending to or focusing on other tasks and other Students.
C. The Light KC cannot make Student activities risk free. The Light KC is not representing it has minimized or will minimize risks of engaging in the Activities.
D. Those participating in The Light KC student activities do so at their own risk.
3. I represent and promise we (the Student and me) will comply with The Light KC policies and the direction given by adults assigned to the Student and/or Activities.
4. We voluntarily assume all risk of Student’s engaging in the Activities and voluntarily waive any related liability of The Light KC and its staff, officers, board members, and volunteers due to their negligence, acts, omissions, or statements. This includes risks incurred while transporting the Student.
5. I consent and give any The Light KC staff member or volunteer leader permission, in any emergency where medical or other treatment is necessary or prudent to make any necessary medical decisions regarding treatment for the Student without the necessity of first notifying me, and to hold harmless any physician, hospital or other medical center for rendering such services. I do understand that if an emergency should occur, reasonable efforts will be made to contact me as soon as possible.
6. We hold The Light KC and its staff, officers, board members, and volunteers harmless from any claims for personal injury, property damage, or illness that may be suffered by the Student, me, any of Student’s family members, or others that arise from Student’s engaging in the Activities, even if due to the negligence, acts, omissions, or statements of The Light KC and/or its staff, officers, board members, other Students, and volunteers.
7. For myself, Student, members of Student’s family and others who may have a claim due to Student’s engaging in the Activities, I promise not to sue The Light KC or its staff, officers, board members, and Students for personal injury, property damage, or illness that may be suffered by Student, me, any of Student’s family members, or others that arise from or during Student’s engaging in the Activities, even if due to the negligence, acts, omissions or statements of due to the negligence, acts, omissions, or statements of The Light KC or its staff, officers, board members, and volunteers.
8. Unless otherwise noted below, I give permission to The Light KC to film/photograph my child(ren) and to use the video/photograph for social media, internet, and print advertisement.
9. I understand The Light KC is relying on and agree it may rely upon this Waiver in allowing Student to engage in the Activities.
I am signing this Student Waiver freely without hesitation or coercion.
